This page provides information and reference materials to accompany the introduction of the revised suite of Drug Interventions Record (DIR) forms.

2007 DIR Background Guidance
This is a reference document for criminal justice drugs workers in the community and CARAT and Clinical workers in prisons, who will be using the revised Drug Interventions Record (DIR)and associated forms (Initial Contact form, Required Assessment form, Activity form) as the key tool for continuity of care and monitoring and research in relation to the Drug Interventions Programme (CJITs and CARATs); and the Substance Misuse Triage Assessment form (prisons only). It includes the background to the purpose of the forms, together with an overview of the changes that have been made since the original DIR was introduced in 2005.

2007 DIR Forms

The new DIR forms are coloured orange and blue to distinguish them from those in use up to 31 March 2007.  Separate Initial Contact and Activity Forms are used by CJITs and CARATs; RA - Assessment Forms are used by CJITs only; the Drug Interventions Record (DIR) and Continuity of Care Update Form (CCU) are used by both CJITs and CARATs. A reference guide, showing when to use which form, can be downloaded here.

Initial deliveries of the new forms (apart from the CCU) have been delivered. Additional supplies will be available to order from 10 April via regional database teams (for non-intensive DATs and CARATs), and from Prolog (for intensive DATs).

Continuity of Care Update Form
The Continuity of Care Update Form (CCU) is a non-mandatory document, the purpose of which is to update personal client information if it has changed significantly from the information recorded on their DIR. This form was created following requests for a standardised form. It is designed to replace existing regional forms which serve the same purpose if required, although DATs, CSPs and CARATs are under no obligation to adopt this form. Unlike the rest of the forms it will not be printed and distributed centrally, so drug workers who wish to use it can download it from here and reproduce it locally.

DIR Review Transition Bulletins

Transition Bulletins are a series of communications about the transition from the old to the new DIR forms. Each bulletin is targeted to the appropriate audience depending on the subject matter and is circulated by e-mail via the relevant network.
